How to Submit FATCA Declaration Form for NPS

NPS account was opened on or after July 1, 2014
employees please send their self declaration form as soon as possible otherwise yours NPS account will be blocked.

• The form is required to be submitted to Central Record keeping Agency (CRA) for NPS at the following address:
        NSDL e-Governance Infrastructure Limited,
        1st Floor, Times Tower, Kamala Mills Compound, Senapati Bapat Marg,
        Lower Parel, Mumbai – 400 013
